Minnesota Masonic Home North Ridge has a rich history of providing quality care to seniors since 1966. We are a specialized senior health care community. Our continuum of care offers a variety of services: Senior Apartments, Assisted Living, Adult Day Program, Complex/Transitional Rehab Care, Long Term Care and secured areas for those with memory impairment. The Physical Therapist will implement with minimal supervision, high-quality physical therapy services for residents, caregivers and facilities in accordance with the principles and practices of physical therapy, State, Federal, and Minnesota Masonic Home North Ridge regulations, policies and procedures.

Duties:

Major job functions include insuring that all residents are afforded the opportunity to achieve and maintain their highest level of function. Maintaining resident care records and accurate billing records, communicating effectively with all necessary disciplines regarding resident progress, problems and approaches to residents. Ensure continuation of appropriate therapeutic follow-up programs. This position works in our Work on Weekend (WOW) program - every other Saturday only, 8am-4:30 pm.

Qualifications:

This position requires a graduate of an American Physical Therapy Association (APTA) accredited program for Physical Therapy. Candidate must possess current licensure to practice Physical Therapy with the State of Minnesota. Experience in adult acute and long term care facility desired but not required. Ability to incorporate all aspects of PT in total resident care as regulations allow. Team philosophy and desire to facilitate long term care residents to their highest level of function. Able to grasp new theories and principles and implement them in practice. Membership in professional organizations desirable.
